ABSTRACT
Introduction:
This research aims to develop an instrument to measure the early adolescent intention to smoke. The behavior of intention is one of the stages of the translation that connects the psychological attitude of an individual's behavior. The intention is seen as the most effective psychological construct to predict actual behavior.

Material and Methods:
This study used a survey method and interviewing a professional. A tool was developed to measure the intention to smoker among early adolescents totaling 16 items with a rating scale of 1-5. The Rasch model was used as it is judged more objective and better able to meet the definitions of measurement in the study.

Results:
The results validity of the instrument shows the value of reliability for the respondents obtained was 0.65. This shows the congruence between respondents with the instruments used. The reliability values for the item is 0.99, which indicates that the instrument has a very good reliability (alpha >0.94).

Conclusions:
Based on research done, a instrument to measure the intention to smoke was created in the form of a scale with reliability so that it can be used to measure smoking intention among young adolescents
